Quick prayers messed up

I had my quick prayers set to Protect item, Piety, and smite, and did the melee tournament, i noticed in the tournament that the only one that worked was piety, then after the tournament while doing bh i died in a fight, while skulled, i was ko'd almost instantly, i died with 70+ prayer points left, and lost my +1. i double checked my quick prayers and they are set to protect item, piety and smite, so i cant understand why i lost my +1, idc about the +1 so much as the fact that i lost it through no fault of my own, please look into this so it doesn't happen to anyone else.
